Home Health Nursing Services

Skilled nursing visits include any of the following:

  • Completion of a procedure requiring substantial and specialized nursing skill such as administration of intravenous therapy, intramuscular injections and sterile procedures
  • Consumer teaching and education/training requiring the skills of a professional nurse.
  • Observation, assessment and evaluation of the physical and / or mental status of the person

Homemaker Personal Care

Homemaker / personal care is the coordinated provision of a variety of services, supports and supervision necessary for the health and welfare of an individual which enables the individual to live in the community.  These are tasks directed at increasing the independence of the individual within his or her home or community.  Homemaker / personal care helps the individual meet daily living needs.

Non-Medical Transportation

Non-medical transportation means transportation to access adult day support, vocational rehabilitation, supported employment community services, as specified by their individual service plan (ISP).
